Interested and qualified applicants should signify their interest in writing. Attach the following documents to the application letter with utmost 3 positions and send to the address below not later than December 21, 2018.

1. Fully accomplished Personal Data Sheet (PDS – CS Form No. 212, Revised 2017 with Work Experience Sheet) which can be downloaded at www.csc.gov.ph NOTE: Submit 10 copies of PDS and WES (1 original copy and 9 photocopies) with 10 original recent passport-sized ID picture (with nametag and white background) and 1 pc. 2 x 2 ID picture. Then, place all documents in just one (1) long brown folder (unfastened).

2. Photocopy of Certificate of Eligibility/Updated and Authenticated Copy of License/Rating by the Professional Regulation Commission;

3. Photocopy of Transcript of Records;

4. Original copies of Certificate/s of Trainings or Seminars for validation.
